For me (New York FIOS), it was as simple as

1. Call FIOS and request that ONT be "provisioned for ethernet"

(I was told it is OK, and that i would lose the ability to order PPV from my cablebox.)

2. Connect an ethernet cable from the ethernet port on the ONT to the WAN port on your DD-WRT router. Check if router obtains WAN IP.

(if it does not, then i don't know...check your cable? find out if ethernet provisioning really is enabled. I have only DATA, NTWK, & MGMT lights green on the ONT. MoCA and POTS are both off.)

3. Have ONT, cablebox, & ActionTec all connected by Coax. You should still have TV reception.

(if you don't, make sure your cablebox is still connected to the ONT. If there's still no reception: check your connections & call FIOS)

4. On your Actiontec:

   a. Disable IP Address Distribution from the Advanced Tab. Save and restart.
   b. Connect an ethernet cable from LAN to LAN on both routers.

5. On your Actiontec from My Network Tab, Network Connections:

   a. Disable all WAN ports. Edit anything that starts with WAN and Disable.
   b. Make sure you have an Ethernet Coax Bridge named Network(Home/Office) 

(if you don't, create a MoCA Ethernet Bridge by clicking Add (the only red word in the My Network Tab). Follow the wizard to create a New Network Bridge between Ethernet and Coax (NOT Broadband Connection Ethernet and Broadband Connection Coax

   c. In the end only three connections will have status enabled: Network(Home/Office) and it's two
      underlying bridged connections, Ethernet and Coax.
   d. Make sure your Network(Home/Office) connection is given an available IP within your LAN's subnet.

6. Power-cycle your cablebox. It should now obtain IP from your DD-WRT router. Widgets and your program guide should work too.

Note: Disable your Actiontec's wireless if you're not going to use it. I already have wifi on my DD-WRT
